Fall is turning out to be a busy season for PCW! Just take a look at our calendar, below, to see what we have going on. I'd love to see you at these events! There are lots of exciting opportunities for PCW members to get involved on the horizon.
Perhaps the most important event we have coming up is our PCW Annual Meeting. Please be sure to mark your calendars for Friday, November 13th. We are excited to have Jose Montero Keynote this year’s meeting as Vice President of Population Health and Health System Integration at Cheshire Medical Center/Dartmouth-Hitchcock Keene. Dr. Montero was formerly the Public Health Director for the state of NH. At D-H, Dr. Montero is developing and directing Population Health strategies by integrating clinical initiatives with community programs. He will be a tremendous resource for us as we plan for our 2016 initiatives. After Dr. Montero’s keynote, you will have the opportunity to learn about all the different ways you can support community wellness as a PCW member. Stay tuned for more details, and register by clicking the link below.
In preparation for our annual meeting, I ask you to please consider nominating an individual or business for a 2015 PCW Award. Is there someone you know who is working hard to improve the health of your community in exciting, collaborative ways? Is there a business that is breaking the mold by promoting the health and wellness of its employees, customers, and community? Learn more about these awards and submit your nomination here.
Lastly, please consider helping our partner, ReThink Health UCRV, in its effort to establish a baseline for our joint work in Advance Care Planning by taking a quick, 2 minute survey. The data, once collected, will be shared with all the wonderful community organizations working to promote Advance Care Planning, and will help us track our progress. I ask each of you to fill it out, and forward it along to others. Information from businesses, schools, and other organizations would be especially valuable. Share this link with at least 3 people, and we will be off to a great start!
Looking forward to seeing you soon.
